Voluntary Benefits

Short-Term Disability*

This coverage provides financial protection in the event you become sick or accidentally injured from a non-work-related accident. Your benefit is 60% of your weekly salary, up to a maximum of $1,000 weekly. The amount of time you must be out of work prior to receiving the benefit is eight consecutive days. Benefits may continue up to 13 weeks. See your plan document for more information.

 

Life & AD&D Insurance

Derstine's provides you with $10,000 of life insurance and accidental death and dismemberment coverage at no cost to you. Please be sure to indicate your beneficiary in Paycom.

Employee Coverage*

  • Coverage amount is a maximum of 5x your annual earnings

  • You may elect $10,000 to $400,000, in increments of $10,000

  • Guaranteed Issue: Up to $100,000, the first time you are eligible for coverage (no health questions required)

 

Spouse Coverage*

  • Coverage amount is a maximum of 100% of the Employee voluntary coverage

  • You may elect $5,000 to $250,000, in increments of $5,000

  • Guaranteed Issue: Up to 100% of your elected spouse coverage or $50,000, whichever is less (no health questions required)

  

Dependent Child Coverage*

  • Coverage amount is a maximum of 100% of the Employee voluntary coverage

  • You may elect $2,000 to $10,000, in increments of $1,000

  • Guaranteed Issue: Up to 100% of your elected dependent child coverage (no health questions required)

  • Full coverage is effective after your child is 14 days old

 

All Life insurance benefits reduce to 65% at age 65 and reduce to 50% at age 70.  Spousal Voluntary life ends when the employee reaches age 70.

Accident insurance provides a cash payout to help cover costs from injuries due to accidents, including medical expenses and recovery. Your benefit amount is based on the type of accident.

*If you choose not to enroll within the first 30 days of your hire date, you can still elect coverage at a future Open Enrollment, but an Evidence of Insurability will be required.
